In order to get you set up, we need the following items completed and sent back to us:
- Shoreline Carrier Information Form
- Shoreline Carrier Agreement
- W-9
- MC Authority Certificate
- Insurance Certificate ($100,000 cargo and $1,000,000 liability required)
